Nuvance Health, formerly known as Western Connecticut Health, has as excellent opportunity for a full-time, experienced Licensed Clinical Social Worker (LCSW) working in an outpatient setting in Norwalk, Connecticut. The LCSW will provide support to the Behavioral and Mental Healthcare department following a Monday-Friday office schedule of 8:30am-5pm with a flexible evening shift once a week. We are located in suburban Fairfield County, about 50 miles northeast of New York City. We offer a highly competitive compensation package including base salary plus a $5,000 sign on bonus.

The ideal candidate will be a seasoned clinical social worker with strong broad based clinical skills and care coordination as well as hold a Connecticut state licensure as a Clinical Social Worker (LCSW).  The position will be providing clinical services to emotionally ill individuals, as well as collaborating with psychiatric and health professionals for the purposes of quality diagnostic and treatment of mental illness, mood, psychotic, and personality disorders with a primary focus on anxiety and depression disorders.


Job Responsibilities include:

  • Conducts interviews with patients/family members to evaluate coping abilities, dysfunctional adjustments, maladaptive behaviors, functional status
  • Establishes objectives and develops patient-focused treatment plans to provide disposition and/or referral
  • Identifies potential barriers, limitations & lack of coping abilities, adjustments, functional status
  • Evaluates patients for mental disorders
  • Provides individual, family, group therapy, crisis intervention and psycho-education.
  • Evaluates for child, elder abuse, domestic violence and makes appropriate referrals
  • Acts as a consultant and/or educator to staff relative to assessment, criteria & referral
  • Maintains resume information and refers patients & families to agencies for supportive services & concrete resources (financial assistance; housing; transportation; medications) on regular & emergency basis
  • Mentors and acts as consultant to peers, staff & community
  • Acts as a liaison for the patients within community (local, state and private agencies)
  • Participates in patient care planning, policy and procedure development, quality performance improvement & projects as assigned by the department manager
  • Provides coverage for absent (vacation, sick, etc.) social workers in medical/surgical, rehabilitation, women's and children's semi areas, emergency & neonatal unit
  • Completes hospital and state required documentation & other paperwork & forms
  • Fulfills all compliance responsibilities related to position, performs all other duties as assigned

Qualifications:

  • Master's degree in Social Work from an accredited school
  • Minimum of 3 years’ experience
  • Current CT Licensed Clinical Social Worker (LCSW)
