As a Social Worker with our team, you will:
Conduct psychosocial and bereavement assessments for home care and hospice patients and their families, serving as a vital resource.
Identify and address patient and family needs, including emotional, social, and financial concerns.
Develop individualized social work plans of care in collaboration with the family and interdisciplinary team.
Provide support and resources to families facing end-of-life care, including bereavement support and referral information.
Connect patients and families with appropriate community agencies and financial resources.
Identify and address patient and family risk factors, ensuring their safety and well-being.
Facilitate effective family and team communication.
Provide in-service education to staff and volunteers.
Maintain timely and accurate documentation that meets Medicare skilled criteria and medical necessity.
Actively participate in team meetings, in-services, and quality assurance initiatives.
Contribute to discharge planning to ensure smooth transitions for patients.
Requirements
What You Need to Bring:
Education: A Master's degree in Social Work (MSW) from a Council on Social Work Education (CSWE) accredited institution.
Experience: Previous social work experience is highly valued.
Communication Skills: Excellent verbal and written communication skills are essential for effective collaboration and patient interaction.
Compliance: Must successfully pass required criminal and driving record checks, as well as a Bureau of Elderly and Adult Services check.
Transportation: Reliable transportation, a valid driver's license, and proof of car insurance with $100,000/$300,000 coverage are required due to frequent local travel.
